Appointment Forms

If you are a new or returning client, you can print and fill out the attached forms prior to our first session. This will reduce the amount of valuable meeting time we spend on paperwork.

All Clients: Please complete the following forms:

1. Client Information Form

2. Adult Clients: Print and sign the Treatment Contract – Adult and Couples

3. Minor Clients: Print and sign the Treatment Contract – Minor Child

4. Review my HIPAA Notice of Privacy Practices and then sign the HIPAA Acknowledgement

5. Diagnostic Intake Questionaire
